The only other detail I can come up with is brake shaft failure. Forgive my ignorance, what is a brake shaft? I know calipers, pistons and discs. Something to do with inboard brakes, no?

I guess you could almost call the drive shafts brake shafts under braking with KERS... but it's a bit weird as a term.
Sorry to be nitpicking. But that looks like inbaord rear and outboard front brakes to me.
Anyways, next question:
The year is 1990. Jean Alesi and Satoru Nakajima are teammates at Tyrrell. Originally Alesi was to sport no.3 on his car and Nakajima no.4. Why did they swap before the season got ahead?

Not at all, Ronnie had a factory-contract with March for 1971 and was just doing a guest appearence for FW at the RoC. However, after the brake-shaft accident, the factory team decided to re-build the cars with outboard brakes for the next race, the also non-championship Questor GP at Ontario Speedway in California, as well as for the rest of the season.
The image you found is interesting for another reason, it shows the 711 with the Alfa-V8, which was raced at the French GP.
The Alfa was a dog of an engine, why the team reverted to the Cosworth for the rest of the season.
